Terms of use

Last updated October 29, 2026.

These terms are an agreement between you and Checkup Score (“we”, “us”), which runs Checkup Score. By creating an account or using the service, you agree to them. Section 10 has an arbitration agreement and a class-action waiver that affect how disputes are resolved. You can opt out of arbitration within 30 days, as it explains.

1. Information, not medical advice

Checkup Score gives general health information. It is not medical advice, and it does not diagnose, treat, cure or prevent any condition. Scores, risk estimates, next steps, answers in Ask and anything else in the app are based on what you and your records tell us and on published guidelines, which may not fit your situation. They don’t replace a clinician who knows you. Talk to a qualified clinician before starting, stopping or changing any medicine, test or treatment, and never delay or ignore medical advice because of something you read here. Using Checkup Score doesn’t create a doctor-patient or other clinical relationship with us.

In an emergency, call 911. If you’re thinking about harming yourself, call or text 988. Nobody at Checkup Score watches what you enter in real time, and we can’t respond to emergencies.

2. The service, and its limits

Checkup Score is new and changing. Features may change, break or stop. We read lab reports, genetic reports and portal records automatically, including with an AI service, and we can make mistakes; that’s why you confirm values before they count and why every step shows its source. Data from devices, patient portals and labs comes from those services, and we aren’t responsible for its accuracy or availability.

3. Your account, and people you add

You must be 18 or older and live in the United States. Keep access to your email secure, since it’s how you sign in, and tell us if you think someone else has used your account.

You may add a profile only for your own child, or for another adult who has agreed to you managing their health information here, or for whom you’re legally authorized to act. You’re responsible for having that permission, and for the accuracy of what you enter. Any adult can take over their own profile at any time.

4. Your data

You own your data. The Privacy Policy and the Consumer Health Data Privacy Policy explain what we collect, why, who processes it and your rights. You can download or delete it at any time.

5. Acceptable use

Don’t use Checkup Score to break the law, to enter someone’s information without permission, to access accounts or data that aren’t yours, to interfere with the service, or to copy, scrape or resell it.

6. Plans, cost and founding membership

Free. Your score and its factors, your first next steps, adding results, your family’s profiles, email reminders and safety alerts are free.

Premium is the full plan for one adult. Family is Premium for two adults and the kids. Each starts with a 14-day free trial (one per person; a card is required) that becomes the plan and payment option you choose when you sign up: 12 months paid monthly (Premium $24.99 a month, Family $44.99), 12 months paid once (Premium $249, Family $449), or month to month (Premium $29.99, Family $54.99). Plans renew automatically on the same term, at the price then in effect, until you cancel; we email you before your trial ends and before each 12-month renewal, and we tell you before any price change applies to you. Before checkout you confirm these terms with a box you tick yourself, and we keep a record of it.

Cancelling Premium or Family. Cancel anytime in Settings and see exactly what happens before you confirm. Cancelling never depends on paying a fee first. During the free trial, your trial ends right away and you aren’t charged. Month to month: no more charges; you keep your plan to the end of the month, with no refund for it. 12 months paid once: no more renewals; you keep your plan to the end of the 12 months, with no refund for the time left, except that within 14 days of a first charge without a trial you can cancel for a full refund. 12 months paid monthly: you’ve agreed to 12 monthly payments, so cancelling before the 12 months end costs an early-cancel fee of $50 or the monthly payments left, whichever is less, charged once, and you keep your plan to the end of the month you’ve paid for. If you keep your plan after all, the fee is refunded. Each new 12 months is a new commitment. Within 14 days of your first charge you can cancel for a refund of that charge instead, with no fee.

Founding membership is optional: $99 a year for one adult, for the first 1,000 members, billed through Stripe. Prices are in US dollars, plus any tax where it applies.

Renewal. It renews automatically each year at the same $99 for as long as you keep your founding membership. We email you before each renewal.

Founding membership refunds. Cancel anytime in Settings, in one step. Within 30 days of any founding charge, including a renewal, you can cancel for a full refund of it in Settings.

What it includes. Founding membership is a membership, not prepaid care. It gives you member pricing (lower prices than non-members pay) on services we offer, such as lab tests, clinician visits and medicines, as each one launches, plus first access to them. Those services are paid for separately each time you use them; the membership doesn’t include them for free. It also includes the app’s paid features for you. We set member prices and may change them, and not every service will have a member price.

Other plans. We may add plans that include more, for example clinician visits or a care navigator, at their own prices. Your founding price applies to founding membership only. You can switch to another plan at that plan’s price; if you do, you leave your founding price behind.

No launch dates. We can’t promise when each service will launch, which is why every founding charge is fully refundable for 30 days.

Referral credit. You can share a personal link to Checkup Score. Someone who doesn’t already have an account and signs up through it within 60 days gets a $20 credit. When they make their first full membership payment (a free trial doesn’t count), you earn a $20 credit, added 60 days later if that payment hasn’t been refunded or disputed. We don’t give credit for accounts that use a throwaway email address or the same card as yours, or for a card that has already earned you credit on another account. Credit has no cash value, can’t be transferred or sold, and is used only toward membership payments to us: at checkout, or on your next renewal if you’re already a member. It doesn’t apply to lab tests, visits, medicines or other services. If a payment that used credit is refunded, the credit isn’t restored.

Share only with people you know, and say that you get a credit when they join (the message we fill in does). Don’t post your link in ads, coupon sites or anywhere you pay for placement, and don’t sign yourself up again. We may remove credit earned from duplicate, fake or self-referred accounts, and we may change or end the program at any time; credit you’ve already earned stays usable for 12 months after a change. We show you only how many friends became members, never who, and never anything about anyone’s health.

7. Other services

Some features connect to services we don’t control, such as patient portals, devices, payment processors, and, where offered, labs and clinicians. Their own terms apply to them. Clinicians and labs you use through Checkup Score are independent and responsible for the care they provide.

8. Disclaimers

To the extent the law allows, Checkup Score is provided “as is” and “as available”, without warranties of any kind, express or implied, including warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement. We don’t promise that the service will be uninterrupted, error-free or secure, or that any score, estimate or step is correct or complete for you.

9. Limits on liability

To the extent the law allows: we aren’t liable for indirect, incidental, special, consequential or punitive damages, or for lost profits or data; and our total liability for all claims about the service is limited to the greater of $100 or what you paid us in the 12 months before the claim. Some places don’t allow these limits, so they may not all apply to you. Nothing here limits liability that can’t be limited by law.

You agree to cover our reasonable costs from a claim by someone else that arises from information you entered about them without permission, or from your breach of these terms.

10. Resolving disputes: arbitration and class-action waiver

Talk to us first. Email hello@checkupscore.com with your name and what you’d like done. We’ll try to resolve it within 60 days, and neither of us will start a case before then.

Arbitration. If we can’t resolve it, you and we agree that any dispute about Checkup Score or these terms will be decided by binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, not in court, except that either of us may bring an individual claim in small claims court, and either of us may ask a court to stop infringement or misuse of intellectual property. We’ll pay the filing and arbitrator fees beyond what you’d pay to file in court, unless the arbitrator finds your claim frivolous. The arbitrator may award the same individual relief a court could.

No class actions. You and we may bring claims only individually, not as a plaintiff or class member in any class, collective or representative proceeding, and the arbitrator may not combine claims. If this class-action waiver is found unenforceable for a claim, that claim goes to court, not arbitration. Where the law doesn’t allow waiving a claim for public injunctive relief, that claim may be brought in court after the individual claims are arbitrated.

Many similar cases. If 25 or more similar demands are filed by or with the help of the same lawyers or group, they’ll be administered in batches of up to 25, with one arbitrator for each batch, and the AAA’s mass-arbitration rules will apply.

Opting out. You can opt out of this arbitration agreement by emailing hello@checkupscore.com within 30 days of first agreeing to these terms, with your name and the words “opt out of arbitration”. Opting out doesn’t affect anything else in these terms.

11. Governing law

These terms are governed by the laws of the State of California and the Federal Arbitration Act, without regard to conflict-of-law rules. Any case that isn’t arbitrated will be heard in the state or federal courts in San Diego County, California, unless the law where you live requires otherwise. Your state’s privacy and consumer protection rights still apply.

12. Ending your account

You can delete your account at any time in Settings. We may suspend or end accounts that break these terms or put others at risk, and we’ll tell you why unless the law or safety prevents it. Sections 1, 8, 9, 10 and 11 continue after your account ends.

13. Changes

We’ll tell you about material changes to these terms at least 30 days before they take effect, by email and in the app. Changes don’t apply to a dispute already started. If you don’t agree to a change, you can delete your account before it takes effect.

14. The rest

If any part of these terms is unenforceable, the rest still applies. Not enforcing a term isn’t a waiver of it. You can’t transfer these terms; we may transfer them as part of a merger or sale, with notice to you. These terms, with the privacy policies, are the whole agreement between us about Checkup Score.
